Before you start
Please take time to check whether you are ready to apply for registration. Please make sure you have the information on the right, some of the items on the list are required, the form will not allow you to proceed if you have not provided them.
Note that you can save the application progress and proceed later.
Checklist of things you need before you start;
- Your NI number
- ID Photo
- Passport/Visa Number
- Copy/picture of Passport/ID
- Proof of address document
- 2 Employment References
- Proof of education – Certificate copies
- Your GP/Doctor’s Details
- Current DBS number if you have one
